Substance Abuse Counselor/Case Managert

Position Description

  • The Substance Abuse Counselor: 
  • Develops and implements treatment plans; provides a variety of interventions including individual and group counseling, crisis intervention, collateral services, and other relevant sobriety-related services to the clients.
  • Documents, reviews and charts client progress and completes all required documents on time
  • Attends treatment meetings and provides input to treatment plan. Prepared treatment and discharge plans and discharge summaries. 
  • Gathers information for intake assessment. Conducts intake interviews and completes individual assessments of client’s status and level of required comprehensive services. Prepares and maintains assessments, comprehensive service plans, progress notes and other records and documentation. 
  • Makes appropriate referrals to community providers coordinating and arranging proposed treatment placements with providers. Facilitates placement including arranging pre-placement visits and transportation.
  • Provides community outreach and early intervention activities by supplying information about the program to community agencies connected to the AOD services system in order to educate them about program availability.
  • Provides support to clients and families and encourages family members and client significant others to participate in client stabilization and counseling services and be able to refer those needing specific services and support. Provides crisis intervention.
  • Assists other staff in dealing with problematic client behavior.
  • Integrates the clinical concept with sound theoretical and practical applications of rehabilitative and individual group and milieu counseling principles.
  • Reviews and charts client progress and completes all required paper work on time 
  • Provides input to develop and improve program servicing and delivery.
  • Displays regular and reliable attendance. 
  • Successfully functions as a team member. 
  • Knowledge of abusive behavior, cognitive therapy techniques, substance-abuse programs and treatment, relapse prevention and 12-Step programs.
     

Education and Experience Requirements

  • A minimum of an Alcohol & Drug Certification plus seven years experience providing Alcohol and Drug counseling to the criminal justice population
  • B.A./B.S. degree in human service field plus two year experience providing Alcohol and Drug counseling to the criminal justice population
