Program Description
Center Star Assertive Community Treatment provides intensive wrap-around services to severely mentally ill adults ages 25 to 59 who have had legal system involvement. In order to qualify for services, clients must have a severe mental illness, have significant deficits in daily functioning and self-care, and must have been treated for their mental illness while in jail sometime during the last year. Program provides medication management, case management, dual diagnosis treatment, vocational rehabilitation, livings skills training, and housing.
Position Description
Provides individual and group rehabilitation to clients dually diagnosed with mental illness and substance abuse.
Provides case management for an assigned group of clients including coordinating and monitoring activities of the treatment team.
Develops, evaluates, and implements treatment goals and plans in conjunction with the treatment team.
Provides individual supportive therapy and symptom management.
Performs psychosocial assessments.
Coordinates the full range of client’s needs. Provides brokerage, linkage and advocacy to ensure resources and services are
accessed in a timely manner, providing community based services in client’s residence as needed.
Conducts individual counseling including crisis intervention, and oversees psycho educational groups.
Prepares critical care issues for discussion during supervision and treatment team meetings.
Provides on-call services as required by contract.

Education and Experience Requirements
Current certification as an AOD counselor (CADC, RAS, etc.)
Bachelors degree in a behavioral science
At least three years’ experience providing clinical services to adults with mental illness.
Minimum age of 25 years.
