Program Description
Families Forward is a Wraparound Program in which mental health professionals and paraprofessionals partner with youth and families to find creative solutions to the family's unique challenges. Wraparound is a family-driven process that facilitates team building and decision making using the strengths and resources of the family and community. Together, the team creates goals to promote stabilization in the home and self-sufficiency through community and family partnership.
Position Description
Wrap Facilitator –
- Provide in-home mental health & crisis intervention services to youth and families.
- Facilitate Wraparound Meetings.
- Complete Assessments and other clinical documents.
- Provide 24-hour crisis intervention as part of team.
- Create & Implement individual client plans.
- Participate in individual and group case consultation.
- Provide required documentation and reports, both clinical and administrative.
- Maintain a flexible schedule according to client needs.
- Provide transportation to client when necessary.
- Create & Implement Psycho-educational groups to clients and families.
- Knowledge of Child development.
- Collaborate with caregivers, community providers and referral sources.
Education and Experience Requirements
- Bilingual Preferred
- Master’s Degree in Behavioral Science
- Registered Intern or Licensure with BBS (IMF/ASW or LCSW/MFT)
- 3-yrs experience with emotionally disturbed or at-risk youth and their families.
