Inter - Case Manager
Job Duties:
Comprehensive Behavioral Assessment, Treatment Planning, Case Management, Crisis Intervention, Facilitation of Treatment Team Meetings, Consultation with Behavioral Coaches, Facilitation of Parent Education groups, all related clinical documentation.
Type of Clients Served:
Children, Adolescents, Transitional aged-youth and Families
Service Delivery: In-Home (Primarily), Residential Treatment Center/Group Home, Day Treatment program, Preschool
Hours Required: 20 hours per week
Type of Supervision: LCSW
